Bilinçli sedasyonun alt gömülü 3. molar dişlerin çekimi sonrası hasta konforu üzerine etkisi

Abstract

Amaç: Gömülü dişlerin cerrahi çekimi sonrası
oluşan ödem, ağrı ve trismusun şiddetinin kontrol
altına alınması için farmakolojik ve/ veya çekim
tekniği ile ilgili birçok yöntem kullanılmış olup
halen de kullanılmaya devam etmektedir. Bilinçli
sedasyon destekli lokal anestezi uygulamaları
hastaların daha konforlu bir operasyon geçirmesini
sağlamakta, ancak postoperatif dönemdeki hasta
konforuna olan katkısı bilinmemektedir. Bu
çalışmada amaç; bilinçli sedasyon uygulamasının
gömülü 3. molar çekimi sonrası oluşan ağrıya ve
ödeme etkisini yalnız lokal anestezi ile
gerçekleştirilen operasyonlarla kaşılaştırmak ve
sayısal verilere dökerek istatistiksel olarak analiz
etmektir.
Gereç ve Yöntem: Çift kör randomize yapılan
çalışmada gömülü alt 3. molar dişlerin cerrahi
çekimi gereken 18- 36 yaş (ort.24,4) aralığında
toplam 40 hasta 2 eşit gruba ayrılarak; ilk grubun
çekimleri bilinçli sedasyon altında lokal anestezi
ile, ikinci grubun ise yalnız lokal anestezi ile
çekimleri yapılmıştır. 6.12. ve 24. saatler de VAS
değerlerine ve 48. saatte ödem miktarlarına
bakılmıştır.
Bulgular: Lokal anestezi ile operasyonları
yapılmış kişilerin ödem miktarları, bilinçli sedasyon
ile operasyonları yapılmış bireylerden istatistiksel
olarak anlamlı derecede yüksek görülmektedir
(p < 0,05) (p=0,000). Bilinçli sedasyon ile
operasyonları yapılan hastaların 12.saat ve
24.saatteki ağrı değerleri yalnız lokal anestezi ile
operasyonları yapılmış hastalara göre istatistiksel
olarak anlamlı derecede düşük görülmüştür
(p < 0,05) (p=0,028) (p=0,036).
Sonuç: Hastaların operasyon sırasındaki ve
sonrasındaki rahatsızlıkları kabul edilir ölçüde
tutmak için bilinçli sedasyon işlemi oldukça
güvenilir bir yöntemdir.
