Abstract

Vertical crown-root fractures (VCRFs) are complex traumatic injuries often deemed non-restorable. Recent advances in biomimetic materials and minimally invasive techniques allow for new treatment possibilities.A 21-year-old female presented with traumatic injuries to the maxillary anterior region. Teeth 11 (right central incisor) and 21 (left central incisor) exhibited complicated crown-root fractures extending subgingivally. Clinical and radiographic assessments, including CBCT, confirmed the diagnosis.Endodontic treatment was initiated. Tooth 21 palatal fragment was reattached using a horizontal fiber-reinforced composite post. A bioceramic sealer was used for obturation. Tooth 11 received a vertical post and both teeth were later restored with zirconia crowns.At the 1 year review, the patient was asymptomatic with no clinical or radiographic signs of failure. Esthetic and functional outcomes were stable and satisfactory.The combined use of bioceramic sealers and horizontal fiber posts can provide a conservative yet effective solution for anterior teeth with subgingival VCRFs, preserving natural dentition and function.
