Pulpal Effects of the Two Office bleaching Materials on Fluorosed Teeth

Abstract

Objectives: This study aimed to evaluate the pulpal effects of 35% hydrogen peroxide used in-office to bleach teeth with dental fluorosis, both in vivo and in vitro. In vivo, histological and clinical effects were assessed. Materials and methods: Ten fluorosed teeth were used for histopathological comparison of two bleaching agents. Additionally, 96 fluorosed maxillary anterior teeth (TFI 3–7) from 17 patients (ages 18–38) were evaluated for bleaching effectiveness and side effects including hypersensitivity, vitality loss, and radiographic changes. In vitro, 90 extracted fluorosed teeth were analyzed for hydrogen peroxide penetration. Data were analyzed using Kruskal-Wallis, ANOVA, and repeated measures ANOVA. Results: Findings showed no irreversible pulpal damage or periradicular abnormalities. Maximum penetration was 16.2 mM for Whiteness HP Maxx and 18.3 mM for Opalescence Xtra. Penetration correlated with fluorosis severity but remained below levels affecting pulpal enzymes. Coclusions: Both bleaching agents were effective and safe for treating mild to severe fluorosis without affecting pulp or periodontium.
