PERKÜTAN ENDOSKOPİK GASTROSTOMİ TÜPÜ OLAN HASTALARDA GÖMÜLÜ TAMPON SENDROMUNUN KLİNİK BELİRTİLERİ VE YÖNETİMİ: OLGULAR EŞLİĞİNDE
Öz
Perkütan endoskopik gastrostomi, ağızdan yemek yiyemeyen ve gastrointestinal sistemi fonksiyonel olmayan hastalarda, enteral beslenmeyi doğrudan gastrointestinal sisteme vermek için tüpün mideye yerleştirilmesi işlemidir. Perkütan endoskopik gastrostomi tüpü uzun süreli beslenme desteği gerektiren hastalar için genellikle tercih edilen bir yoldur. Perkütan endoskopik gastrostomi tüpüne bağlı majör komplikasyon görülme oranı %1-4 iken minör komplikasyon oranı ise %8-30’dur. Perkütan endoskopik gastrostomiye bağlı minor komplikasyonlar major komplikasyonlara oranla üç kat daha fazla görülmektedir. Perkütan endoskopik gastrostomi tüpünün majör komplikasyonlarından gömülü tampon sendromu, perkütan endoskopik gastrostomi tüpü iç desteğinin (tampon, plaka) mide ve karın duvarındaki gastrostomi kanalından çevresine doğru hareket ederek herhangi bir yere yerleşmesi durumudur. Gömülü tampon sendromuna bağlı olarak gastrointestinal kanama, perforasyon, peritonit, karın içi ve karın duvarı apsesi gibi komplikasyonlar meydana gelebilmekte ve bu komplikasyonlar ölümcül sonuçlara sebep olabilmektedir. Gömülü tampon sendromu insidansının yaklaşık %1 olduğu tahmin edilmektedir. Gastrostomi tüpünün dış ve iç plakasının çok sıkı olması, gömülü tampon sendromuna yol açan esas etiyolojik faktör olarak kabul edilmektedir. Tanı endoskopik veya transabdominal ultrason ile konulmaktadır. En önemli önleyici tedbir ise plakanın yeterli konumlandırılması ve bakım sırasında plakanın 360 derece saat yönünde döndürülmesidir. Bu yazıda, gömülü tampon sendromu olgular eşliğinde ele alınmış, gömülü tampon sendromuna dair bilinmeyenler ve önleme stratejileri literatür sonuçlarıyla derlenmiştir.
