Kalsifiye Epitelyal Odontojenik Tümör Oluşumuna Bağlı Olarak Gömülü Kalmış Dişin Cerrahi Tedavi Sonrası Ortodontik Sürdürülmesi

Öz

Kalsifiye Epitelyal Odontojenik Tümör oldukça nadir görülen lezyonlardandır. Literatürde neoplazm ya da kist olarak tanımlandığı gibi intraosseoz veya ekstraosseoz gibi çeşitli şekillerde sınıflandırılmıştır. Bu lezyonun histopatolojik olarak en belirgin özelliği kist epitelinde bulunan ve “hayalet” olarak adlandırılan karakteristik hücrelerin bulunmasıdır. Her yaşta ve her cinsiyette eşit oranda görülebildiği gibi genellikle mandibulada premolar bölgede görülürler. Genellikle gömülü bir diş ile ilişkilidirler. Asemptomatik ilerler ve kemikte büyüme, dişlerde rezorbsiyonlara neden olabilirler. İlerlemiş vakalarda radyolüsent görüntünün içinde düzensiz radyoopak adacıklar izlenebilmektedir. Literatürde genellikle iyi huylu olarak ifade edilmesine rağmen bildirilen nüks ve malignite vakaları vardır. Lezyonun yeri ve büyüklüğüne bağlı olarak çeşitli tedaviler olsa da genellikle enükleasyon tercih edilmektedir. Bu vaka raporunda 13 yaşında kadın hastada asemtomatik olarak ilerleyen kalsifiye epitelyal odontojenik tümörün tedavisi ve tümörün çevresinde gömülü kalan mandibular ikinci premolar dişin ortodontik sürdürme tedavisi sunulmaktadır.

Orthodontic Maintaining after Surgical Treatment of Impacted Tooth due to Formation of Calcified Epithelal Odontogenic Tumor

Öz

Calcified Epithelial Odontogenic Tumor is one of the rare lesions. It has been classified in various ways such as intraosseous or extraosseous, as well as defined as neoplasm or cyst in the literature. Histopathologically, the most distinctive feature of this lesion is the presence of characteristic cells called “ghosts” in the cyst epithelium. Although it can be seen at all ages and in all genders, it is usually seen in the premolar region of the mandible. They are often associated with an impacted tooth. They progress asymptomatically and may cause bone growth and tooth resorption. In advanced cases, irregular radiopaque islets can be observed in the radiolucent image. Although it is generally expressed as benign in the literature, there are reported cases of recurrence and malignancy. Although there are various treatments depending on the location and size of the lesion, enucleation is generally preferred. In this case report, the treatment of an asymptomatically progressive calcified epithelial odontogenic tumor and the orthodontic maintenance treatment of the mandibular second premolar tooth impacted around the tumor in a 13-year-old female patient are presented.
