The Effect of Oral Feeding Interventions on Growth of Infants with Cleft Lip and Palate: A Systematic Review

Öz

Introduction: Feeding is the most common problem experienced among the infants with cleft lip and palate. Especially weight gain and growth of these infants are the criteria that plays a role for correction surgically. Therefore, nurses have important tasks in feeding management of these infants.
Aim: We aimed to investigate the previous randomized controlled studies focusing on the effect of nutritional interventions on the growth of the infants with cleft lip and palate in this systematic review.
Method: We searched for randomized controlled studies with available full-text studies published between 1979 and 2019 on PUBMED and CINAHL databases. Preferred Reporting Items for Systematic Reviews (PRISMA) standards were used for systematization.
Results: Systematic review of literature revealed 174 published international studies. 45 and 124 studies were discarded due to duplication and non-randomized study, leaving 5 studies focusing the effect of different nutritional modalities and interventions on growth parameters. In the studied sample 288 were included. Parental nutritional consultation, obturators, modified pacifiers, rigid or squeezable baby bottles and their effects on weight gain were examined in 5 studies. Influences of upright positioning during feeding, burping, and limitation of feeding to 30 minutes, breastfeeding were examined in 2 studies, spoon-feeding, syringe and cup use were studied in one study.
Conclusion: Previous studies are insufficient to provide a significant effect of nutritional interventions on growth of infants with cleft lip and palate, emphasizing the need for further research suggesting to perform on larger sample sizes and randomized trials to determine the effect.

Dudak Damak Yarıklı Bebeklere Uygulanan Oral Beslenme Girişimlerinin Büyümeye Etkisi: Sistematik Derleme

Öz

Giriş: Dudak damak yarıklı bebeklerin doğum sonrası sıklıkla yaşadıkları sorunlar arasında beslenme ilk sırada yer almaktadır. Cerrahi olarak dudak damak yarığının düzeltilmesinde bebeklerin vücut tartı artışı ve yeterli büyüme kriterleri rol oynar. Bu nedenle dudak damak yarığı olan bebeklerin beslenme yönetiminde hemşirelere önemli görevler düşmektedir.
Amaç: Dudak-damak yarıklı bebeklere uygulanan beslenme girişimlerinin bebeğin büyümesine etkisini belirlemek amacıyla gerçekleştirilen bu sistematik derlemede randomize kontrollü çalışma sonuçları incelendi.
Yöntem: İncelenen araştırmalar PUBMED ve CINAHL veri tabanlarında, yöntemleri ve bulguları PRISMA standartları doğrultusunda sistematize edilerek 1979 - 2019 yılları arasında seçim kriterlerini oluşturan İngilizce yayınlanmış, randomize kontrollü klinik araştırmalar çalışma kapsamına alındı.
Bulgular: Sistematik inceleme sonucunda dudak damak yarıklı bebeklerin beslenmesi ile ilgili 174 uluslararası yayına ulaşıldı ve 45 çalışma duplikasyon, 124 çalışma seçim kriterlerini sağlamaması nedeniyle dışlandı. Dudak damak yarıklı bebeklerin oral beslenme yeteneğini arttırmada farklı yöntem ve uygulamalar kullanarak beslemenin büyüme parametreleri üzerine etkisini değerlendiren beş araştırma incelendi. İncelenen araştırmalarda örneklem grubuna toplam 288 bebek dahil edilmiştir. İncelenen araştırmalarda annelere beslenme danışmanlığı, damaklık, modifiye edilmiş emzik, standart biberon, sıkıştırılabilir biberon kullanımı; iki araştırmada ise dik pozisyonda besleme, gaz çıkartma ve beslenme süresini sınırlandırma (30 dakika), annenin emzirmesini sağlama, bir araştırmada ise enjektör, kap ve kaşıkla beslenme yöntemlerinin vücut tartı artışına etkisinin incelendiği görüldü.
Sonuç: Araştırmalar incelendiğinde, beslenme girişimlerinin büyümeyi anlamlı düzeyde etkilediğine yönelik kesin kanıtlara ulaşılamadı. Bu sistematik inceleme sonucunda, konuya ilişkin daha büyük örneklem grupları ile farklı beslenme girişimlerinin büyümeye etkisini belirleyebilecek randomize kontrollü klinik çalışmaların yapılması gerektiği önerilmektedir.
