Aim: The objective of this case report is to present a successful root canal treatment of a maxillary first molar having three mesiobuccal canals with separate orifices.
Case presentation: An 18 year old female patient was admitted to our clinic, complaining from severe pain. We observed during the clinical examination that root canal treatment of the maxillary right first molar was already initiated. The tooth was percussion sensitive, cold and heat sensitive, although there was no referred pain. A broken endodontic instrument in the disto-buccal root canal was found during the radiographic examination. After extensive clinical and radiographic examination tooth was prepared for nonsurgical endodontic treatment. Starting the endodontic treatment, it was observed the presence of two canals orifices in the buccomesial root with no preparation. Though we failed to remove the fracture for the endodontic instrument, the instrument was bypassed. Root canals were obturated with guttapercha and sealer. The tooth was restored with composite resin.
Conclusion: Recall examination as far as 6 months post-treatment found no sensitivity to percussion or palpation and recall radiography after treatment show no evidence of any periradicular lesion.
Amaç: Bu olgu raporunun amacı, ayrı kanal ağızları bulunan üç meziyobukkal kanallı bir maksiller birinci büyük azı dişinin başarılı endodontik tedavisini sunmaktır.
Olgu bildirimi: 18 yaşında bayan hasta, şiddetli ağrı şikâyetleriyle kliniğimize başvurdu. Klinik muayenede 16 numaralı dişin kök kanal tedavisine daha önce başlanıldığı gözlendi. Diş perküsyona, soğuğa ve sıcağa karşı duyarlılık gösteriyordu. Radyografik muayenede distobukkal kök kanalında kırık bir endodontik alet tespit edildi. Derinlemesine kliniksel ve radyolojik muayeneden sonra diş endodontik tedaviye alındı. Başlayan endodontik tedavi ile dişte hiç preparasyon yapılmamış iki meziyobukkal kanal ağzı varlığı görüldü. Endodontik alet kırığı kanaldan çıkarılamasa da aletin yanından geçilerek apikal daralıma ulaşıldı. Kök kanalları gutta percha ve sealer ile dolduruldu. Diş kompozit rezin ile restore edildi.
Sonuç: Tedaviden 6 ay sonraki takip randevusunda perküsyon ve palpasyon hassasiyeti olmadığı gözlendi ve bu randevuda alınan takip radyografisi herhangi bir periradiküler lezyon varlığını göstermedi.

First issue of the Journal of Cumhuriyet University Faculty of Dentistry was published in 1998. In 2010, journal's name was changed as Cumhuriyet Dental Journal. Journal’s publication language is English.
CDJ accepts articles in English. Submitting a paper to CDJ is free of charges. In addition, CDJ has not have article processing charges.
