Actinomyces infection related with maxillary sinus

Abstract

Actinomyces are anaerobic Gram-positive prokaryotic bacteria belonging to the family Actinomyceataceae. They can be found in calculus, periodontal pockets, carious lesions, oral mucosal surfaces, upper respiratory tract, gastrointestinal, and female genital mucosa. Approximately 55-60 % of actinomycotic infections in human are originated cervicofacial region. A. israelii is the main species responsible for the human disease. Actinomycosis involving the maxilla usually is seen as a localized intraoral infection in contrast to classical cervicofacial actinomycosis. Formation of draining sinuses, local swelling, and pain are the most common presenting symptoms. We report a case of actinomyces infection that was localized maxillary molar tooth and association with maxillary sinus in a 39 years old woman. After seven days antibiotic therapy surgical curettage therapy was performed to patient. During monthly recalls for 3 months no recurrence was noted and the infected area has been healed completely.

Maksiller sinüsle ilişkili aktinomiçes enfeksiyonu

Abstract

Aktinomiçes; Actinomyceataceae ailesinde yer alan anaerobik gram pozitif prokaryotik bir bakteridir. Diş taşında, periodontal cepte, çürük lezyonlarında, oral mukozal yüzeylerde, üst solunum yollarında, GİS’de ve kadın genital mukozasında bu mikroorganizmaya rastlanabilir. Aktinomiçes enfeksiyonlarının % 55-60’ında etken, servikofasiyal bölge kaynaklıdır. İnsanlardaki ana sorumlu olan tip ise A.israelii’dir. Klasik aktinomiçes enfeksiyonlarından farklı olarak maksillayı içine alan aktinomiçes vakaları generalize olmayıp lokalize yerleşimlidir. Sinüslerden drenaj, ağrı ve lokal şişlik en sık rastlanılan semptomlardır. Bu vaka 39 yaşında bayan hastanın, sol maksiller azı dişleri bölgesinde yerleşimli ve maksiller sinüs ile ilişkili aktinomiçes enfeksiyonu sunulmaktadır. Hastaya 7 günlük antibiyotik tedavisini takiben cerrahi küretaj uygulanmış olup, hastanın 3 aylık takibinde herhangi bir nüks olmadığı ve bölgenin tamamen iyileştiği tespit edilmiştir.
