POSTERİOR SUPERİOR ALVEOLAR ARTERİN KIBT İLE DEĞERLENDİRİLMESİ

Öz

Amaç: Maksiller posterior dental alan, periapikal cerrahi, implant yerleştirme ve sinüs lift gibi cerrahi uygulamalar açısından önemli bir alandır. Posterior superior alveolar arter (PSAA), maksiller sinüsün lateral duvarında bulunur ve bu tür cerrahi prosedürler sırasında yaralanabilir. Bu nedenle, bu bölge ile ilgili bilgi, klinisyenler açısından önem arz etmektedir. Bu çalışmanın amacı, PSAA’in maksiller sinüsün lateral duvarı ile olan ilişkisinin ve prevalansının belirlenmesidir.
Gereç ve Yöntem: Çalışmamızda 300 hastadan çeşitli nedenlerle alınan Konik Işınlı Bilgisayarlı Tomografi dataları retrospektif olarak değerlendirilmiştir. Kesitsel görüntülerde PSAA’in vasküler kanalının görünürlülüğü, eğer bulunuyorsa arter çapları, yükseklikleri, alveoler kret tepesine dik uzaklıkları ve kret tepesine olan mesafeleri belirlenmiştir.
Bulgular: Çalışmamızda arter çapının genel olarak 1-2 mm arasında olduğu gözlemlenmiş olup, 2 mm’in üzerinde olduğu vakalara az rastlanmıştır. Ayrıca erkeklerde sol tarafta bulunan alveoler krete dik uzaklık ve mesafe ölçümleri (p<0.05) kadınlardan daha yüksek bulunmuştur.
Sonuç: Daha başarılı cerrahi girişimler için intraossöz anastomozlarının ve maksiller sinüs morfolojisinin, preoperatif olarak KIBT ile değerlendirilmesi büyük önem taşımaktadır.

Evaluation of the Posterior Superior Alveolar Artery Using CBCT

Öz

Aim: The maxillary posterior dental area is an important area for surgical applications such as periapical surgery, implant placement and sinus lift. The posterior superior alveolar artery (PSAA) is located in the lateral wall of the maxillary sinus and can be injured during such surgical procedures. Therefore, information about this region is important for clinicians.
The aim of this study is to determine the relationship of PSAA with the lateral wall of the maxillary sinus and to determine its prevalence.
Materials and Method: In our study, Cone Beam Computed Tomography data obtained from 300 patients for various reasons were evaluated retrospectively. In cross-sectional images, the visibility of the PSAA's vascular canal, artery diameters, heights, perpendicular dis-tances to the alveolar crest and their distances to the crest were determined.
Results: It was observed that the artery diameter was generally between 1-2 mm, and cases with more than 2 mm were rare. In addition, left-sided perpendicular distance to the alveolar crest and distance measurements (p<0.05) were found to be higher in male than in female.
Conclusion: We conclude that preoperative imaging with CBCT seems to be helpful for assessing the location of the PSAA and the maxillary sinus morphology, which may be used to adjust the surgical treatment plan to yield more successful dental implant treatments.
