Decompression Method of Treatment for Dentigerous Cyst Associated with Impacted Lower Second Molar Tooth

Abstract

Dentigerous cysts are the most common cysts in the maxilla and mandible after radicular cysts. They are more common in the mandible and men. They are usually symptomless. Patients present with the complaint of unerupted teeth and they are noticed during routine radiography examination. On radiographs, they are generally observed as wide radiolucency around the crown of an unerupted tooth. CBCT (cone-beam computed tomography) is the golden key in the evaluation of the relationship of cysts and tumors with anatomical structures. In advanced cases, dentigerous cysts enlarge by damaging adjacent bone and tooth structures and may lead to conditions that affect the comfort of life such as function, phonation, or fracture. Although cyst enucleation is among the treatment options, it is an ideal treatment to add marsupialization, decompression, and cyst reduction to enucleation in advanced cases. In this case report, the diagnosis and treatment of a dentigerous cyst associated with an impacted tooth, located close to the mandibular canal, which poses a risk of paresthesia and fracture, is described in a young patient.

Gömülü Alt İkinci Molar Dişle İlişkili Dentiregöz Kistin Dekompresyon Yöntemi ile Tedavisi

Abstract

Dentigeröz kistler radiküler kistlerden sonra maksilla ve mandibulada en çok görülen kistlerdir. Mandibulada ve erkeklerde daha sık görülürler. Genelde semptom vermezler. Hastalar dişlerin sürmemesi şikâyeti ile başvurular ve rutin radyografi muayenesinde farkedilirler. Radyografilerde genel olarak sürmemiş bir dişin kronu etrafında geniş radyolüsensi olarak izlenirler. KIBT (konik ışınlı bilgisayarlı tomografi), kist ve tümörlerin anatomik yapılarla olan ilişkinin değerlendirmesinde altın anahtardır. Dentigeröz kistler, ilerleyen vakalarda komşu kemik ve diş yapılarına zarar vererek genişlerler ve fonksiyon, fonasyon veya fraktür gibi hayat konforunu etkileyen durumlara yol açabilirler. Kist enükleasyonu tedavi seçenekleri arasında olsa da ilerlemiş vakalarda marsupyalizasyon, dekompresyon yöntemi ile kistin küçültülmesi yönteminin enükleasyona eklenmesi ideal bir tedavidir. Bu vaka raporunda genç bir hastada gömülü bir diş ile ilişkili, mandibular kanala yakın yerleşimli, parestezi ve fraktür riski oluşturan dentigeröz kistin tanı ve tedavisi anlatılmaktadır.
