KRONİK BOYUN AĞRILI HASTALARDA FARKLI SOLUNUM TİPLERİNİN SOLUNUM PARAMETRELERİ ÜZERİNE OLAN ETKİSİ

Abstract

Amaç: Kronik boyun ağrısı olan hastalarda solunum parametrelerinde azalma olduğu belirtilmiştir.
Ancak farklı solunum tiplerinin solunum ile ilişkili parametreler üzerine etkisi kapsamlı olarak
incelenmemiştir. Bu çalışmanın amacı, kronik boyun ağrısı olan hastalarda farklı solunum tiplerinin
solunum parametreleri üzerine olan etkisini incelemekti.
Yöntem: Kesitsel araştırma yöntemi ile yapılan çalışmaya kronik boyun ağrısı tanısı alan 52 hasta
(yaş= 31,66±10,96 yıl) alındı. Solunum tipi üst-kostal tip (n=30) ve kosta-diyafragmatik tip (n=22)
solunum yapan hastalar üst göğüs solunum grubu (ÜGSG) ve diyafragmatik solunum grubu (DSG)
olarak ikiye ayrıldı. Bireylerin ağrı düzeyi, Görsel Analog Skalası ve özür düzeyi, Boyun Özürlülük
Anketi ile değerlendirildi. Solunum fonksiyonları (FVC: zorlu vital kapasite, FEV1: birinci saniyedeki
zorlu ekspirasyon volümü, FEV1/FVC oranı, PEF: tepe akım hızı, FEF%25-75: zorlu ekspiratuar volümün
% 25-75 değeri, VC: vital kapasite ve MVV: maksimum istemli ventilasyon) spirometre, inspiratuar
ve ekspiratuar solunum kas kuvveti (MIP ve MEP) ağız basıncı ölçüm cihazı ile ve solunum kas
enduransı sabit eşik yükte IMT cihazı kullanılarak değerlendirildi.
Sonuçlar: Dinamik akciğer volümleri ve VC değerleri gruplar arasında benzer bulundu (p>0,05).
MVV (p=0,026), MIP (p=0,041) ve solunum kas enduransı (p=0,044) ÜGSG’de DSG’ye göre anlamlı
düzeyde düşüktü.
Tartışma: Kronik boyun ağrılı hastalarda üst kostal tip solunum, kosta-diyafragmatik tip solunuma
göre MVV, MIP ve solunum enduransı değerlerinde azalmaya neden olmaktadır. Kronik boyun ağrılı
hastalarda boyun bölgesine yönelik nöromotor kontrol egzersizler ile solunum tipini düzenlemeye
yönelik egzersizler veya stratejilerin solunum parametrelerine etkisi araştırılmalıdır.
