The Complications of Space Maintainers and Space Regainers in Pediatric Dentistry

Abstract

Early loss of primary teeth may cause problems such as loss of arch length, teeth crowding, ectopic eruption, impaction of permanent teeth, cross-bite formation and midline shift, thereby increase the need for orthodontic treatment. The use of space maintainer and space regainer appliances can prevent and reduce the severity of malocclusion that occurs with the early loss of primary teeth. The localization, type and number of primary teeth require different types of space maintainers to be designed. The space maintainers designed as removable, fixed, semi-fixed, functional, non-functional also have different complications depending on the usage, material or design. Major complications include dental caries, infection, plaque accumulation, local pain and discomfort, soft tissue injuries, unconvinient movements and positioning in the underlying permanent tooth, fracture, decementation, and patient related loss of the appliance. In this review, the informations were given about the classification of space maintainer and space regainer appliances, the types of appliances and possible complications that may be encountered.

PEDODONTİDE YER TUTUCULARIN VE YER KAZANDIRICILARIN KOMPLİKASYONLARI

Abstract

Erken süt dişi kaybı, ark uzunluğunun kaybı, çapraşıklık, ektopik sürme, daimi dişlerin gömülü kalması, çapraz kapanış oluşumu ve orta hat kayması gibi sorunlara yol açabilir ve ortodontik tedavi ihtiyacını artırır. Yer tutucu ve yer kazandırıcı apareylerin kullanımı süt dişlerinin erken kaybıyla ortaya çıkan ve gelişmekte olan malokluzyonun şiddetini azaltabilir ve önleyebilir. Kaybedilen süt dişlerinin lokalizasyonu, tipi, sayısı farklı türde yer tutucuların dizayn edilmesini gerektirir. Hareketli, sabit, yarı sabit, fonksiyonel, non-fonksiyonel olarak tasarlanan yer tutucuların kullanım kaynaklı ya da yapım materyali veya dizaynına bağlı olarak farklı komplikasyonları da bulunmaktadır. Ana komplikasyonlar arasında diş çürüğü, enfeksiyon, plak birikimi, lokal ağrı ve rahatsızlık, yumuşak doku zedelenmeleri, alttan gelen daimi dişte uygun olmayan hareketlere ve pozisyonlanmaya neden olma, apareyde kırılma, desimantasyon, kaybolma gibi komplikasyonlar sayılabilir. Bu derlemede yer tutucu ve yer kazandırıcı apareylerin sınıflandırılması, aparey çeşitleri ve bu apareylerde karşılaşılabilecek olası komplikasyonlar hakkında genel bilgiler verilmiştir.
