Healthy ranges for Alanine Aminotransferase levels and affecting factors: The prevalence study in Tokat
Abstract
Background and Aims: Determination of the upper limit of the normal range (ULN) for alanine aminotransferase (ALT) is important, especially in hepatitis and non-alcoholic fatty liver. We aimed to determine the normal transaminase levels and their relationship with effect factors in a northern province of Turkey. Materials and Methods: This study was conducted in 70 (12 urban, 58 rural) areas in the province of Tokat with about 530,000 inhabitants aged 18 years and older. The study population of 1,095 subjects (male/female=541/554; urban/rural=555/540) was selected by a random sampling method. In all, 217 of the 1,095 individuals, who did not have risk factors for ALT, were considered to be healthy individuals. The ULN listed by the test manufacturer for ALT was 55 U/L. Results: The mean age of the 1,095 individuals was 41.4 ± 17 years. The ALT levels were correlated with aspartate aminotransferase (AST), total cholesterol, triglycerides, and body mass index (BMI) in men, women, and both. In 217 individuals (126 male, 91 female), serum ALT levels for the mean, 5th, 75th, and 95th percentiles were 36.1 ± 8.8, 26.0, 39.0, and 54.2 U/L, respectively. Giriş ve Amaç: Karaciğeri etkileyen hastalıkların doğru değerlendirilebilmesi
için normal alanin aminotransferaz (ALT) seviyesinin üst sınırının belirlenmesi oldukça önemlidir. Bu çalışmada Tokat ili erişkinleri’nde
normal ALT seviyesinin ve bu seviyeyi etkileyen etmenlerin belirlenmesi
amaçlanmıştır. Gereç ve Yöntem: Bu çalışma Tokat ili’ne
bağlı 70 merkezde (12 ilçe merkezi ve 58 kırsal bölge) yapıldı. 18 yaş ve
üzeri nüfusu 530.000 kişi olan ilden 1.095 kişi (erkek/kadın=541/554;
kent/kırsal=555/540) rastgele örnekleme yöntemiyle seçilerek çalışmaya
dahil edildi. 1.095 kişinin 217’sinde ALT düzeyini etkileyecek herhangi
bir risk etkeni saptanmadı ve bu kişiler sağlıklı katılımcı olarak değerlendirildi.
Çalışmada kullanılan ALT kitinin normal üst sınırı 55 U/L’ydi.
Bulgular: 1.095 kişinin ortalama yaşı 41,4±17 yıldı. ALT seviyesi erkekte,
kadında ve tüm katılımcılarda AST, total kolesterol, trigliserit seviyesi
ve beden kitle indeksiyle koreleydi. 217 katılımcıda (126 erkek ve
91 kadın) serum ortalama ALT seviyesi, 5., 75. ve 95. persantiller sırasıyla
36,1 ± 8,8, 26, 39 ve 54,2 U/L bulundu. 91 kadının köyde yaşayanlarında
ALT seviyesi şehirde yaşayanlardan yüksekti (p < 0,043). Sonuç:
Toplumumuzda ALT’nin üst sınırı günümüzde kullanılan değerlere benzer
bulunmuştur.
